I acknowledge that, by accessing and using IRIS, I may create, access, use, or disclose personal information and confidential information of our customers and stakeholders.

I understand that I am required to comply with:

• The Privacy and Personal Information Protection Act 1998 (NSW).
• The Health Records and Information Privacy Act 2002 (NSW) (where applicable).
• Relevant NSW Government policies, code of conduct, and LLS privacy, information management and ICT policies.

I acknowledge that personal information must be:

• Used, disclosed or modified only where authorised or for a lawful purpose related to my role,
• Accessed only on a need-to-know basis,
• Accurately recorded, securely stored, and protected from unauthorised access or disclosure.

I understand that:

• Unauthorised access to, misuse of, or disclosure of information may constitute a privacy breach,
• Suspected or actual privacy incidents must be reported immediately in accordance with LLS procedures,
• Breaches may be subject to investigation and may result in disciplinary action, termination of access, and/or legal consequences.
